As a thing, it's a kind of church. In Germany every big cathedral church is called a "dom". In the Netherlands the only dom is the one in Utrecht, housing (previously) the archbishop. In Italy it's the main church in town.

In the S&M community, dom is a shortened form of dominatrix.
